The Cardiovascular Thoracic Surgery Team is looking to add a team member who values team collaboration and has a passion for helping others. The Care Coordinator - RN works to maximize the patient’s health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator – RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families). Care Coordinator - RN Overview Key responsibilities include but are not limited to: Coordinates care across the health care continuum within designated sub-specialty population(s) Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services Completes pre-visit planning and post visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visit Facilitates proactive disease management and preventative health care using evidenced based practice Performs other duties as determined by manager and clinic needs Minimum Qualifications: Current Minnesota RN license and minimum of ASN BLS certification 3 years of clinical nursing experience Demonstrated experience in patient advocacy and education, coaching, and proactive disease management EMR and keyboarding/computer skills Clinic Description: Southdale Cardiology Clinic Location: 6405 France Ave S., Edina, MN 55435 Hours: 40 hours a week M Physicians offers competitive salary and excellent benefits and provides you with the opportunity to enhance your career in the exciting field of health care through rewarding and challenging assignments and the opportunities for advancement.
