As a Care Coordinator – RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum. Our Fairview Southdale Cardiology Clinic supports a wide variety of patients with their cardiac needs. This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more. We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care. Southdale was rated #1 in the twin cities for cardiac care, and coronary intervention. Hybrid working schedule (some remote and in clinic time).* The Care Coordinator - RN works to maximize the patient’s health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator – RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families).
