Position Summary The primary purpose of your position is to execute the treatment plan of the Physical Therapist to improve and/or maintain LifeCircles participants' mobility level and other functional abilities. This position is responsible for assisting in the overall physical therapy program at LifeCircles. This includes providing therapeutic exercise, gait training, DME coordination, and the planning and development of group exercise programs for maintenance therapy of LifeCircles participants under the direction of the Physical Therapist.

- Executes the plan of therapy to include exercise, treatments, institution of adaptive aids and/or maintenance with goals for participants when indicated.

- Provide services to participants in the day center and community, including homes and facilities. 

- Participate in development and implementation of movement challenges, groups, education, and activities, as indicated. 

- Monitoring and maintaining DME for proper fit and use.

- Provides training to caregivers, family members, friends, for the applicable area of the treatment plan.

- Reviews restorative care plans with the PT’s as directed by department policy.

- Participates in the coordination of the participant’s 24-hour delivery of care services.

- Participates in Quality Improvement activities as directed by supervisor 

- Complete documentation in accordance with organizational standards 

- Uses appropriate safety measures when providing care and performing other job duties.

- Supports participant autonomy and dignity by communicating in a respectful and sensitive manner according to their needs.

Required Qualifications 

  • Must have successfully completed and associates degree program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). Must possess a current, unencumbered, active license as a physical therapy assistant in the state of Michigan.
  • One year of experience working with the frail elderly
  • Must have reliable transportation and a current valid Michigan driver’s license.
  • Must possess current CPR certification by AHA or be willing and able to obtain as offered by LifeCircles.
  • Strong desire to work in a team based model of care and excellent communication skills 
  • Organized and have excellent time management skills 
  • Proficient in Excel, Microsoft Office, Outlook, and electronic medical records
  • Strong sense of professionalism, good critical thinking, creativity, flexibility, and excellent problem-solving skills
  • High ethical standards and comfortable in self-directed daily work
