FLSA: Non-Exempt, Hourly
Status : Full-time, 40 hours/week

Schedule : Monday-Friday, Day Shift ***NO WEEKENDS***

Geriatric appropriate clinical services are provided both in the onsite clinics and in participant’s homes. Position Summary

The primary purpose of your position is to provide LifeCircles participants with active and engaging activities that promote, cognitive, physical, social, emotional, and spiritual wellness. Programming will be center around participant interests, will be designed to support health and functions in efforts to support participants remaining in the community.

Required Qualifications

  • One year of experience working with the frail elderly
  • High School Diploma or GED "Experience in activity programming and facilitation preferred"
  • Must have reliable transportation and a current valid Michigan driver’s license.
  • Must possess current CPR certification by AHA or be willing and able to obtain as offered by LifeCircles.
  • Strong desire to work in a team based model of care
  • Excellent communication skills
  • Organized and have excellent time management skills
  • Proficient in Excel, Microsoft Office, Outlook, and electronic medical records
  • Strong sense of professionalism, good critical thinking, creativity, flexibility, and excellent problem-solving skills
  • Comfortable in self-directed daily work

Required Qualifications Position Functions/Duties

  • Participates in the planning of monthly programming
  • Consults with the recreation team before changing scheduled programming
  • Understand participant's plan of care for any specified impacts to programming
  • Prepares for and facilitates daily programming as scheduled
  • Understands and adheres to participant diets when providing programming that include food and fluids
  • Invites and assists participants to programming
  • Uses Positive Approach to Care (PAC) approaches to assist with positive and effective engagement with participants
  • Make adaptations to programming as needed to maximize participant engagement
  • Maintains accurate and current documentation of groups, observations and interventions
  • Reports changes in a participant’s condition to the supervisor or Day Center Manager immediately
  • Makes suggestions to the supervisor regarding changes in a participant’s plan of care based on a participant’s expressed wishes.
  • Obtains supervisory approval for the purchase of equipment and supplies
