Position Summary: The primary purpose of this position is to provide leadership, resources, and support to our day center nurse aide team and home care department in the daily delivery of care and service. The Day Center Supervisor is an actively engaged member of the Interdisciplinary Team and plays a critical role ensuring that care and services align with the PACE Model of care and in accordance to the participant Life Plan and goals of care. This position will work closely with other departments in providing high quality day center and home care experiences supportive of participant's overall health and function.
Required Qualifications
High School Diploma or GED required
Minimum of 2 years working in a health care environment is required.
Leadership, supervisory, and health care related program oversight responsibilities, required.
Home care experience, required.
Must have reliable transportation and a current valid Michigan driver’s license.
Must possess current CPR certification by AHA or be willing and able to obtain as offered by LifeCircles.
Minimum of one year of experience working with the frail elderly, or long-term care population
Skills and Knowledge
Must have strong organizational, critical thinking, and problem-solving skills
Must have strong interpersonal skills
Must be technologically adept and proficient in electronic medical records
Must posses excellent written and verbal communication skills
