United Hospice & Palliative Care of Arizona is growing and looking to expand our Nursing Team!

JOB DESCRIPTION SUMMARY

The Registered Nurse (RN) Case Manager is a professional member of the interdisciplinary group who plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health and education/experience. The Registered Nurse (RN) builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care

Complete an initial, comprehensive and ongoing assessment of patients and families to determine hospice needs.

Record a complete physical assessment and history of current and previous illness(es).

Provide professional nursing care by utilizing all elements of the nursing process.

Assesses and evaluates patients’ status by:

  • Writing and initiating a plan of care.
  • Regularly re-evaluating patients and families/caregivers needs.
  • Participating in revising the plan of care as necessary.

Initiate the plan of care and make necessary revisions as patients’ status and needs change.

Use health assessment data to determine nursing diagnosis.

Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.

Include the patients and the families/caregivers in the planning process.

Initiate appropriate preventive and rehabilitative nursing procedures.

Administer medications and treatments as prescribed by the physician in the physician's plan of care.

Counsel the patients and families/caregivers in meeting nursing and related needs.

Provide health care instructions to the patient as appropriate per assessment and plan.

Assist the patients with the activities of daily living and for optional comfort care.

Act as Case Manager when assigned by Clinical Director and assume responsibility to coordinate patient care for assigned caseload.

Communication

Complete, maintain and submit accurate and relevant clinical notes regarding patients’ condition and care given.

Record pain, symptom management changes, and outcomes as appropriate.

Communicate with the physician regarding the patients’ needs and reports changes in the patients’ condition

Obtain physicians' orders as required.

Communicate with community health related persons to coordinate the care plan.

Teach the patients and families/caregivers self-care techniques as appropriate.

Provide medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

Provide and maintain a safe environment for the patients.

Assist the patients and families/caregivers and other team members in providing continuity of care.

Work in cooperation with the families/caregivers and hospice interdisciplinary group to meet the emotional needs of the patients and families/caregivers.

Additional Duties

Participate in on-call duties as defined by the on-call policy.

Ensure that arrangements for equipment and other necessary items and services are available.

Supervise ancillary personnel and delegate responsibilities when required.

Assume responsibility for personal growth and development regarding professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

Fulfill the obligation of requested and/or accepted case assignments.

Participate in quality assessment performance improvement activities.

Attend required company meetings.

Complete necessary training per licensure and the company.

Other duties as assigned.

KNOWLEDGE AND ABILITIES

Must have knowledge of disease processes, including end of life concerns.

Must operate with cultural sensitivity with respect to working with people and groups of diverse, racial, ethnic, economic, and social backgrounds.

General computer skills required.

Experience with EMR systems preferred.

SKILLS

POSITION QUALIFICATIONS

Active Registered Nurse (RN) License in state of Arizona

CPR Certification

Fingerprint Card

Must be a licensed driver with an automobile that is insured and in accordance with the state/and or organization requirements and is in good working order.

WORK ENVIRONMENT

Prolonged or considerable walking or standing.

Able to lift, position, and/or transfer patients.

Able to lift supplies and operate equipment, as necessary.

Considerable reaching, stooping, bending, kneeling, and/or crouching.

Visual acuity and hearing to perform required nursing skills.

Skilled Nursing Facilities (SNFs)/Group Homes and personal residence.

Considerable driving and in/out of vehicle.

Possible exposure to infectious diseases.

Pay: $78,000.00 - $88,000.00 per year
