· We are looking for Amazing RN Case Managers who demonstrate compassion and have a hospice heart. Experience with Home Care Home Base a plus, but will train. This is a FULL TIME position.

· We have dedicated on call staff and weekend staff so our RN case managers just work Mon-Fri day shift.
