Description

POSITION OVERVIEW:

Provides case management and direct staff support to foster stability and overall well-being of participant families.

ESSENTIAL FUNCTIONS:

ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • In accordance with the vision, mission and values of Hushabye Nursery, the Family Coach is responsible for providing quality care and services that meets the needs of persons served.
  • Completes pre-screening assessments on new referrals.
  • Assists with scheduling intake assessments with the Social Worker or Trauma specialist
  • Facilitates tours of the facility to new families.
  • Ensures client’s enrollment needs are completed including but not limited to coordination of care with OBGYN and hospital, completion of ROIs, coordination with hospital for transfer of care, and completion and sending care letters to outside providers.
  • Conduct strengths-based needs assessment with each family on case load and develops family partnership agreements with participants, creating measurable goals to ensure families are successful in accomplishing their identified goals.
  • Provides care and services in a supportive, compassionate, and non-judgmental manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.
  • Collaborate with local programs and assist participants in accessing community resources and support services. Ensuring having up-to-date information and locating specialized resources as needed.
  • Collaborate with Department of Child Services to assist participants in meeting goals and applicable requirements, as needed.
  • Timely and accurately complete documentation including utilizing an online database in accordance with the established guidelines, procedures, and HIPPA.
  • Coordinate, under Administrative Director approval, all financial requests.
  • Inform and assist participants in enrolling into Hushabye programming.
  • Initiate and facilitates community partnerships.
  • Participate in teaching and facilitating classes for participants and support at program events/activities.
  • Works collaboratively with the peer mentors, trauma specialist, social worker, and other team members
  • Maintain and uphold Standards of Practice.
  • Report child abuse and/or neglect as in accordance with Arizona State law.
  • Any additional projects or tasks as assigned by senior leadership.
  • May require flexible hours to include some evenings, weekends and/or on call coverage.
  • Completes required orientation and continuing competencies as required by policy and procedure.

Requirements

 KNOWLEDGE, SKILLS, AND EXPERIENCE:

  • Experience working with Medication Assisted Treatment, Opiate Use Disorder, Substance Use Disorder and Department of Child Safety preferred.
  • Expert knowledge of case management and healthcare management principles.
  • Experience working with transdisciplinary health teams.
  • Effective communication, organizational and time-management skills.
  • Background in health care, social work, mental health, and/or substance use.
  • Problem solving skills and ability to multi-task.
  • Must be passionate about treating families who are struggling with substance abuse.
  • Ability to help foster a trauma informed, inclusive environment for all mothers, families, and support systems.
  • Bilingual in Spanish preferred
  • Current AZ DPS Level 1 fingerprint clearance. (MUST UPLOAD FINGERPRINT CARD WITH APPLICATION)

EDUCATION (MINIMUM REQUIREMENTS):

  • Bachelor’s Degree in Social Work, Education, or other related field. Equivalent experience as outlined below can serve in lieu of Bachelor’s degree.
  • 2 years of experience in behavioral health or other related field.
