United Cerebral Palsy of Central Arizona is Hiring a HCBS Program Coordinator Near Phoenix, AZ

HCBS Coordinator provides assistance to Program Manager with supervision and day to day support to the Direct Care Workers (DCW) and the delivery of an assigned caseload of members to ensure best practices, quality of care and effective communication. Program Coordinator assists Program Manager in overall administration of the program.

Essential Functions

  • Ensure services are according to UCP’s mission, vision and value statements.
  • Demonstrates an understanding of members served rights, protection and safety.
  • Supervise the work of Direct Care Workers providing support, coaching and feedback.
  • Trains Direct Care Workers on department procedures, orient Direct Care Worker to member’s basic needs and diagnosis.
  • Ensure all staff are trained in appropriate behavior management techniques (Article 9 training and Prevention/Support) with all members.
  • Monitor Direct Care Workers are current on DDD, agency, and departmental trainings and monitoring requirements.
  • Act as liaison between families, UCP, and direct care staff to ensure quality service delivery and continuity of service. Reports to HCBS Manager any issues families or Direct Care Workers may have.
  • Provide QA monitoring as specified by DDD requirements. This includes 90 day ATC monitoring, staff supervisions and any home visits needed per DDD regulations.
  • Prepare and attend ISP meetings, includes participation in development of the ISP Plan, and writing outcomes and teaching strategies with input from the member and family. Ensure ISP documentation is sent to DDD and Family/guardian.
  • Follow up with DDD support coordinator or family to confirm ISP meeting schedule.
  • Monitor implementation and accurate completion of member’s ISP outcomes according to individual’s support plan.
  • Assure completion of all intake and outtake materials. Provides information regarding programming to families and providers on every level.
  • Conducts interviews and hires qualified candidates.
  • Assist with disciplinary issues with Program Manager /Director and/or HR involvement.
  • Oversee, Direct Care Workers schedules, ensuring shifts are covered and there is no GAP in service provision.
  • Provide coverage to prevent a GAP in service for members.
  • Demonstrate knowledge of contractual, legal and regulatory requirements.
  • Monitor the maintenance of department operations to achieve compliance according to agency and department policies and procedures, DDD monitoring, OLCR Certification, and state and federal licensing, standards, and regulations.
  • Ensure members’ files and records are in compliance with agency policies, procedures, and state regulations. Members’ information is scanned into web-based software system, create and maintain hard copy file of member’s information.
  • Contribute to all aspects program service delivery by interacting with members and being an active participant in all related activities.
  • When notified of any staff or member health and safety issues/concerns to immediately take action, report and document any reported health and/or safety issues including but not limited to, basic first aid, serious injuries/illnesses, emergency situations, hospitalizations and complete reports and notify appropriate personnel (Manager, Director, HR, etc. ) as necessary.
  • Communicate effectively and professionally with members, support coordinators, direct care workers, co-workers, ISP Team, and member’s families.
  • Demonstrate and model a professional presence (appropriate language, initiative, respect, punctuality, manners, maturity, and consistency) and appearance.
  • Handle difficult situations with a positive attitude by actively seeking problem resolution, and creative solutions.
  • Report suspected or alleged abuse or neglect as required by UCP policy, DDD and state law.
  • As necessary, complete, process and report incident/accident reports accurately, within timelines established in agency, department and DDD policies and procedures.
  • Assist in the Wildly Important Goals (WIGs) process, ensuring weekly commitments are completed within timelines.
  • Protect and maintain the confidentiality of member information and documentation in accordance with UCP policy, HIPAA, state and federal law.
  • Review and ensure accurate record keeping (monthly outcome reports, surveys, travel time timesheet, program reports, mileage logs, attendance sheets, toileting charts, medication records, skin integrity logs, ATC monitoring reports, Personal Care task analysis checklist, seizure record, training timesheet, etc.) within established timelines.
  • Complete ATC monitoring’s according DDD regulations and timelines.
  • Monitor that medication is properly administered and recorded in accordance with law and as prescribed by licensed medical professionals and according to UCP medication training.
  • Review completed habilitation outcomes for accuracy, ensuring teaching strategies and data collection is accurate and processed to DDD within timelines.
  • Participate in department strategic plan, collecting information and data for program results.
  • Assist with processing timesheets to payroll, ensuring that related documentation required by staff are accurately completed, including signatures of DCWs and parent/guardian and processed to payroll within timelines
  • As needed, complete and process facility, vehicle or IT (Information Technology) maintenance requests according to timelines.
  • Maintain optimum attendance and reliability.
  • Review billing software and Focus for current authorization.
  • Communicate with DDD Support Coordinator to update authorization for payment.
  • Update billing software system with current member and staff information.
  • As requested, may serve as a back-up for the HCBS Manager and Direct Care Workers.
  • Perform other related duties as assigned.
  • Qualifications

Minimum

  • Associates degree in a Social Service or related field with working with individuals who are developmentally or physically disabled.
  • Must have two years of verifiable supervisory experience
  • Must have at least one year of verifiable experience working in the DDD community or more than one year as a direct care worker providing services to individuals who are developmentally or physically disabled.
  • Current knowledge in Arizona DDD regulations and procedures is mandatory.
  • Must be able to function independently have time management skills with the ability to work a flexible schedule which may include nights and weekends.
  • The ability to work effectively with members, providers, office staff, agencies and a diverse population.
  • Must possess the ability to make independent decisions when circumstances warrant such action.

Job Type: Full-time

Pay: $40,000.00 - $42,000.00 per year
