Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Worcester.

PASSagesis an intensive, short-term (2-4 week), inpatient treatment program for men and women 18 and older who suffer from substance use disorders but do not require detox. The program provides individual counseling sessions and comprehensive case management services to each client along with at least seven treatment groups per day, run by all team members. PASSages is an exciting, dynamic environment in which all members of the multidisciplinary team work together to ensure the best outcomes for our clients. PASSages Case Managers work collaboratively with clients to ensure they have what they need to maintain their sobriety and achieve their treatment goals. This includes helping them resolve any pending legal matters; helping them set up and get to important medical or mental health appointments; helping them navigate probation, parole, DCF and other systems in which they may be involved; and helping to set up further treatment for them to participate in once they leave PASSages, including such things as outpatient appointments, intensive outpatient programs (IOP's), long term residential programs (LTRP's), and sober living houses.

Responsibilities

  • Ability to implement program policies and procedures in order to maintain daily structure and consistency within the program milieu and maintain compliance with DPH licensing standards.
  • Ability to effectively communicate and develop positive professional relationships with staff and co-workers and outside professionals and complete all required documentation.
  • Knowledge of and edibility to work effectively with community-based resources, referral networks, and public/private systems of care.
  • Ability to assess basic physical, social, educational, legal, and service needs of a client and coordinate care with other inter-agency and community-based providers.
  • Specific Duties:

  • Work with treatment team to provide needed services to each client assigned to the case manager.
  • Make referrals and assist clients with transition to behavioral treatment providers including outpatient services, residential placement, and self-help groups.
  • Coordinate and provide transportation as needed.
  • Attend program staff and clinical team meetings, supervision, trainings, and all other required program meetings.
  • Conduct outreach activities at appropriate referral sites.
  • Lead informational groups with clients on a regular basis.
  • Clearly and consistently document all client interactions, and work done on behalf of clients, in the program's electronic health record.
  • Respectfully intervene with any problem client behaviors to help clients improve and grow.
  • Perform other related job duties.
  • Duties may include the assignment of Tobacco Education Coordinator or Human Rights Coordinator.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.

Qualifications

  • Grade C02: Case Manager I: Must have HSD/GED or HiSet; Associates degree preferred
  • Grade C04: Case Manager II: Requires a Bachelor's degree; preferably in the field of psychology or human services
  • Certification in addictions counseling or minimum of 1 (one) year of experience in field required.
  • This position requires transporting clients as an essential function of the role. A current, valid US Driver's license and a review of your motor vehicle record is required for a determination of your eligibility to drive for this work-related purpose. A CHL vehicle may be available but if you will be using your personal vehicle you will be required to provide proof of current registration and inspection.
  • Knowledge of the Massachusetts continuum of care for substance abuse.
  • Understanding of addiction and recovery.
  • Desire to serve others required.
