Schedule Details:

Monday through Friday, On Call - Not Required, Weekends - Every Eighth Weekend

TheBSAS School-based Programprovides substance use disorder prevention services to students in their learning environment in public and private schools within Central Massachusetts. We offer assessment, social skills training, mentoring, family support, recreational activities, case management and consultation, and connection to community resources. Community Healthlink staff uses a nurturing and disciplined approach to help at-risk youth and teens deal with the issues that can distract them and prevent them from succeeding in the classroom.

Responsibilities:

  • Develop, implement, and review and Individual Service Plan (ISP) to meet the identified needs of each youth in an effort to effectively and appropriately coordinate service delivery.

  • Provide regular mentoring sessions that include teaching, coaching, and supporting development of a range of skills in youth.

  • Maintain client records (including completing Intervention plans, progress notes, and discharge summaries).

  • Communicate with collaterals for the purpose of treatment planning.

  • Participate in case consultation meetings.

  • Conduct psycho-educational groups with youth and family members.

  • Ability to assess basic physical, social, educational, legal, and service needs of client and coordinate care with other inter-agency and community-based providers.

  • Attends staff supervision, meetings and trainings as required.

  • Follow CHL protocols.

  • Some evening and weekends may be required.

  • Travel is required as part of this position.

Qualifications:

  • Grade C04: Case Manager II: Requires a Bachelor or Master's degree; preferably in the field of psychology or human services

  • Minimum of one to two years working with children, adolescents and families with behavioral, social and/or emotional difficulties.

  • Proven ability to effectively engage clients, successfully advocate for clients and to work well with other professionals.

  • Comfortable providing home-based and community based services

  • Demonstrated strong work ethic, ability to work independently and ability to contribute to team objectives

  • Demonstrated strong communication skills

  • Ability to use a computer-based information system.

  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work-related purposes including transporting clients in a personal vehicle

  • Must be able to pass a CORI background check.
