TheTenancy Preservation Program (TPP) provides a clinical presence in the Central MA Division of the Housing Courts and short-term intensive case management to eligible households whose tenancy problems are related to a disability. Our primary responsibility is to the Housing Court, providing professional assessment and recommendations regarding the disability-related causes of the tenancy problems and recommendations for service plans and other reasonable accommodations to address the tenancy problems.

Case Managers Supervisor provides supervision to TPP Case Managers and works as a member of the TPP Management Team to provide short-term case management to eligible households whose tenancy problems are related to a disability.

Responsibilities:

  • Supervises TPP Case Managers

  • Maintains required supervisory documentation.

  • Participates as a member of the TPP management team.

  • Provides trainings for case managers.

  • In collaboration with the Program Director, collaborates with stakeholders within Community Healthlink and in the community to improve access to TPP services.

  • In collaboration with the TPP Program Director, creates workflows for effective and responsive teamwork among case managers.

  • In collaboration with the TPP Program Director, assists in tracking and reporting monthly data to funders and the Housing Court

  • Identify gaps in services and provide data for the purpose of client advocacy.

  • Participate in collaborative case conferences regarding service planning for clients.

  • Participates as a member of the Community Healthlink Management Team

  • Regularly attends staff meetings, team meetings, supervision and all other required meetings.

  • Maintain a caseload of clients facing eviction.

  • Develop service plans to address issues effecting tenancy and complete referrals for on-going services.

  • Works closely with tenants, service providers, landlords, and housing court personnel

  • Maintains knowledge base of services to assist in prevention of homelessness.

  • Performs other related duties.

  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.

Qualifications:

  • Supervisory experience required.

  • HSD/GED/HiSet/ASSOC/Required, Bachelor's degree or higher preferred.

  • Bilingual candidates are urged to apply.

  • Client-focused and patient-centered.

  • Ability to multitask.

  • Strong interpersonal skills.

  • Self-motivated and self-directed.

  • Knowledge of substance use disorder, mental health, primary care, and related health concerns, familiarity with community resources and entitlements.

  • Strong analytical skills, including the ability to recognize problems, think creatively, and devise innovative solutions.

  • Experience with Outlook Suite (Outlook, Word, Excel, etc.).

  • Familiarity with client documentation, electronic medical records
