Provides a range of client-centered services that link client with health care, psycho-social education and other services that insures timely, coordinated access to medically appropriate levels of health, mental health and support services, and continuity of care. Key activities include on going assessment of the client’s needs and personal support systems, client-centered health education and advocacy, and review of utilization of services.

Minimum Education: Some College

Education Notes: Some college experience. AA degree preferred.

Minimum Experience: Experience working in a health care setting. Experience with customer service (dealing effectively with the public). Experience working with children and families with a culturally diverse population.

• Lift/Push/Pull: 25 lbs, over 25# with assistance or equipment
