Summary of Essential Functions for the Position:
Committed to Caring , the Hospice Scheduling Coordinator is responsible to ensure efficient scheduling of the clinical staff, patient care activities and communication with team members regarding scheduling activities and communication within the EMR system, complying with State and Federal regulatory and agency guidelines. This position ensures proper and accurate information is entered timely, and will assist with patient, staff, physician, referral source phone calls, providing backup to intake processing referrals, and support the Director of Nursing and Executive Director with administrative duties to include but not limited to IDG, billing, ordering of medical supplies, and customer care calls.

Educational Requirements:
High School Diploma or GED Equivalent Required; 2 year Associate Degree preferred.
Working Conditions & Physical Requirements:
Working environment is quiet and can be stressful due to deadlines, multiple tasks and general compliance of law, rules and regulations. The position requires visual acuity and dexterity, sitting, standing, some pushing, pulling, and lifting up to 25 pounds.
