Treasure Health is Hiring a PRP Coordinator Near BALTIMORE, MD
Treasure Health Systems is currently seeking a PRP coordinator to implement the rehabilitation activities outlined in the Clients Individual Treatment Plan (IRP). This position is supervised by the Rehabilitation Specialist and provides direct services to clients to include intake assessments, individual / family / rehabilitation group treatment and crisis intervention.
Duties and Responsibilities:
PRP coordinators are required to assess the needs of the client throughout their time in the program
Provides services on a weekly basis, in the home, office or community or remotely.
Completes all notes (direct and in-direct) on our EMR (electronic medical record software) daily.
Support the Rehabilitation Coordinator to Complete treatment plan updates in our EMR for each client that details the client's progress towards goals, response to services and goals that were the focus of the services, the client's response to the services and a follow-up plan of action
Attends weekly team meetings and training as scheduled.
Participates in monthly supervision with the Rehabilitation Specialist.
Collaborates with the client's assigned mental health therapists monthly and documents the type of collaboration (i.e. phone call, email, attending a meeting, etc.).
Other duties as assigned
Additional Requirements:
Daily and/or weekly home visits to clients’ homes (currently telehealth services)
Use of own vehicle for visits to clients’ homes
Valid driver’s license
*Must have experience in PRP working with children and adults
*Naloxone training
*CPR training
*DLA-20
JOB REQUIREMENTS: Basic Qualifications
A bachelor's degree in a health-related field.
An associate degree in a health-related field.
30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; or 1 year of work experience in a supervised mental health setting or PRP experience is preferred.
