Job Summary:
The Registered Nurse (RN) works under the direction of the Nurse Manager and under direct supervision of the Charge Nurse on duty each shift. The RN works in collaboration with all other clinical care departments to provide safe high quality nursing care to patients as assigned. The RN utilizes knowledge of basic nursing principles, practices, and ethics in order to perform procedures and tasks within their scope of practice.
The RN effectively utilizes the nursing process to assess, plan, implement and evaluate patient needs to assure age and developmentally appropriate nursing care for patient population served (infants, children, adolescents and young adults). The RN is responsible for providing and documenting nursing care, in accordance with developed care plan and physicians orders for each individual patient in a safe patient and family-centered process.

About Totally Kids Rehabilitation Hospital
Requirements:
Current California Registered Nurse (RN) license as issued by the State of California
Bachelors of Science degree in nursing (BSN) preferred
Current American Heart Association Basic Life Support (BLS), Pediatric Advance Life Support (PALS)
Current Advanced Cardiovascular Life Support (ACLS) certification (acute only)
Certified Rehabilitation Registered Nurse (CRRN) preferred
Certified Pediatric Nurse (CPN) (preferred)
Minimum of two (2) years of pediatric nursing experience in a subacute or acute care hospital preferred
Documented completion of the hospitals preceptor program (40 hours) within two months of hire
Three (3) or more years of previous experience pediatric and/or rehabilitation Nursing (preferred)
Ability to prioritize accordingly and maintains a high level of organization
Serves as a role model and provides positive influence to facilitate change when needed
Values life-long learning and regularly participates in ongoing clinical training and education
Competently recognizes and supports the needs of a culturally diverse workforce and patient population
Ability to lift thirty (30) pounds without assistance
Ability to communicate effectively in English, both verbally and in writing
Must possess good work ethics and a professional image at all times
Knowledge of CMS conditions of participation, CDPH state regulations, CCS, and TJC (hospital, nursing care center, and laboratory) accreditation requirements as applicable to job
Working knowledge of computer and software applications as applicable to job

SPECIFIC FUNCTIONAL RESPONSIBILITY:
A. Assessment and Planning
Plans, organizes, prioritizes daily work using sound judgment and completes tasks in allotted time before the end of each shift
Initiates, updates, and documents age appropriate plan of care in collaboration with interdisciplinary team, patients, and families based on assessments and goals to ensure quality and continuity of care
Participates in interdisciplinary team care conferences as assigned
Performs patient assessments/reassessments and collaborates with physicians and other care team members to develop appropriate plans of care, and nursing interventions as appropriate
Initiates and updates short-term plans of care appropriately
Performs and documents complete patient assessments for new admissions and readmissions and assures accuracy in the initiation of new patient orders, medical record, and plan of care
Plans meal and rest breaks according to policy; collaborates with the charge nurse and other team members to provide safe coverage to assigned patient workload
B. Medication and Treatment Safety
Recognizes and participates in medical emergency responses according to hospital policy and scope of practice; ensures accuracy and completeness of documentation of patient outcomes
Applies medication and treatment safety practices by using the six rights to medication and treatment administration: right patient, right medication/treatment, right route, right dose, right time, right approach
Collaborates with pharmacy staff to ensure accurate and timely medication delivery on new/refill medications
Participates in and ensures that medication and treatment orders are received, processed, transcribed and reconciled timely and accurately
Holds self-accountable to maintain knowledge of appropriate techniques and care of patients of various ages, including infant, toddler, adolescent and young adults according to hospital policy and best practice
Maintains competency in administering and preparing IV medications, TPN solutions and fluids safely according to hospital policy and best practice
C. Medical Equipment and Point of Care Testing:
Manages, operates, inspects and tests nursing equipment to ensure that it is functioning safely and efficiently prior to use and removes nonfunctioning equipment from use according to hospital policy
Uses age-appropriate techniques to obtain specimens and correctly process laboratory and diagnostic testing, and evaluates and supports the competency of others as appropriate
Performs, interprets and takes appropriate action on waived testing and results (point-of-care testing) per hospital policy
Maintains competency in point of care equipment according to hospital policy and training
D. Documentation and Reporting:
Provides and documents nursing care accurately and concisely in accordance with nursing practice and hospital policy
Evaluates, responds appropriately, and documents timely in medical record patients vital information, signs and symptoms of illness, and therapeutic interventions as appropriate
Provides accurate and organized hand-off communication report using the SBAR system, to oncoming Licensed Nursing Staff (LNS), Charge/Resource nurses as needed, physicians, and whenever leaving the care of patients to another to ensure patient safety and continuity of care
Follows through, as needed, on information given by shift report, patient or family regarding patient concerns
Documents and reports changes in patient condition and outcomes of medical emergencies through approved methods, hospital policy, and expectations
Completes patient care documentation accurately, legibly, and in a timely manner
Recognizes abnormal laboratory values/diagnostic reports and notifies the Charge Nurse and/or physician promptly
Maintains a thorough knowledge of all departmental documentation requirements and appropriate forms/logs to be used
E. Patient Care Area-General
Engages patients with a positive and friendly demeanor, while providing patient and family centered care
Adheres to hospital infection control policies and current best practices
Practices all care and duties using safe body mechanics
Participates in training and orientation of new personnel as assigned
Participates in patient care clinical rounds as an active member of the clinical care team
Participates with internal/ out of facility transportation of patients requiring assistance of a Registered Nurse
Participates in the quality assurance and performance improvement (QAPI) process through auditing and reporting as assigned by the Nurse Manager
Demonstrates self-reliance and uses critical thinking to solve complex problems quickly and efficiently
Demonstrates adaptability to working overtime and altering work schedule when priority nursing care affects immediate patient care
Oversees patient care provided by the Certified Nursing Assistant (CNA) during assigned shift
Consistently involves patient and family in planning for care, treatment, teaching needs and discharge plans
