The Lead Case Manager is responsible for coordinating case management and safe and timely release services, provides guidance related to patient flow, discharge process and training new case managers, and supervising the work of other case managers.
The Lead Case Manager will provide clinical case management services focused on enhancing patient-centered care and maximizing outcomes across from the patient care continuum from admissions to post-discharge. The Case Manager will collaborate with the clinical team to assist in development, planning, coordination, and administration of the activities of clinical review, discharge planning, resource utilization.
The incumbent will carry-out her/his duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital. The incumbent will have knowledge of health care systems, and have demonstrated ability to interact, relate to, work with, and support the activities of a diverse workforce.
