The Lead Case Manager is responsible for coordinating case management and safe and timely release services, provides guidance related to patient flow, discharge process and training new case managers, and supervising the work of other case managers.

The Lead Case Manager will provide clinical case management services focused on enhancing patient-centered care and maximizing outcomes across from the patient care continuum from admissions to post-discharge. The Case Manager will collaborate with the clinical team to assist in development, planning, coordination, and administration of the activities of clinical review, discharge planning, resource utilization.

The incumbent will carry-out her/his duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital. The incumbent will have knowledge of health care systems, and have demonstrated ability to interact, relate to, work with, and support the activities of a diverse workforce.

Primary Responsibilities

  • Monitor and screen for daily eligible referrals, assign, and monitor case managers caseload.
  • Foster a professional and welcoming environment in all interactions with clients, guests, vendors, and colleagues, regardless of differing identities, backgrounds, beliefs and choices.
  • Manage and evaluate the performance of assigned staff in collaboration with the Manager.
  • Develop and maintain professional and supportive relationships with staff and subordinates.
  • Work in tandem with case managers to assist clients in reaching their case plan goals.
  • Assumes a leadership role and collaborates with the interdisciplinary team to manage care, through criteria driven processes, for the appropriate level of care, patient status, discharge needs and resource utilization.
  • Evaluates admissions for medical necessity using approved criteria at defined intervals throughout the episode of care.
  • Provides compassion to the patient and the patient’s family members.
  • Develops and ensures delivery of patient care including assessment, planning, implementation, and evaluation of patient outcomes.
  • Revises plan of patient care as needed.
  • Assesses patients’ post discharge needs and facilitates the provision of services necessary to meet identified needs. 
  • Facilitate referral and linkage process to appropriate services.
  • Attend staff meetings as needed.
  • Other duties as assigned.

Education and Experience

  • Required: Associates degree in Nursing from an accredited school of nursing or 
  • Required: Bachelor’s degree in related field such as social work
  • Preferred: Bachelor’s degree in Nursing from an accredited school of nursing
  • Required: 5 years Case Management experience in a health care environment
  • Preferred: 5 yearsof experience in a health care environment
  • Preferred: Experience with electronic and case management documentation systems; willing to train
  • Required: Meet all Employee Health Requirement

 License/Certification(s)

  • Required: Current RN or Social Work licensure in the State of New Mexico. 
  • Required: Current BLS certification
