Who we are looking for: The new Care Manager/Guardian Coordinator will live in Las Cruces, have experience in case/care management, exhibit advanced communication and interpersonal skills, and share our vision of providing service excellence through collaboration with our clients, their families and other community service & health care providers. This person has the ability to meet their clients' goals while working independently as well as with our strong team of professionals.
Why are we looking for the right person: We are growing in Las Cruces. We feel that in order to meet the growing needs of the senior population, we need a Care Manager in Las Cruces.
Who is that person: We require 2-3 years of Care/Case Management experience with disabled adults and/or seniors. Bachelor's/Masters in Nursing, Social Work or Related Field required. Experience in Hospice, Home Care, Hospital, Case Management or Community Care Management preferred. You must be able to work independently and be flexible to work with cognitively impaired adults, their families, the courts and various professionals. Mental Health experience a plus.
