The primary duties of the Patient Navigator are to coordinate the care of patients in Latina Clinic. The job involves working with patients, physicians and support staff to meet patient goals successfully. The primary responsibility of a patient navigator is to maintain a high degree of excellence in clinical outcomes while providing effective coordination of patient care.

• Answer patient calls and text messages during quot;reasonablequot; hours.
• Remind patients about appointments; work directly with schedulers
• Assist patients in navigating their care (finding the pharmacy, lab, etc)
• Assist patients in applying for medical assistance and other necessary documentation
• Connect patients to community services
• Direct liaison between patients and providers to answer medical questions
