SCOPE OF RESPONSIBILITY:

Problem-Solving is a humanistic approach that works to reduce system in-flow by creatively assisting households and individuals in maintaining their current housing or identifying an immediate and safe housing alternative.

Under the leadership of the Director of Housing Navigation, the Problem-Solving Coordinator will work in a mezzo capacity to integrate the implementation of the Problem-Solving Intervention throughout the Family Solution Center. The Family Problem-Solving Coordinator will carry a caseload of no more than 10 households at any given time.

JOB DUTIES:

  • Implementing the Problem-Solving intervention throughout the Family Solution Center
  • Working with case managers in interim housing settings to support the integration of Problem- Solving in their work
  • Work interdepartmentally to assess problem solving/diversion funded positions and prioritize areas of training and technical assistance to support positions
  • Ensuring all FSC staff have been officially trained on Problem-Solving
  • Working with the LAHSA Problem-Solving Unit to streamline PSAF requests and intervention updates
  • Ensuring Problem-Solving Tracking is happening across the FSC
  • Working with key partner family systems to integrate Problem-Solving further upstream from the Coordinated Entry System to reduce in-flow
  • Coordinate, facilitate, train and support problem solving/diversion providers in post-award activities in conjunction with the system components team, such as: new contract onboarding, new program kick-offs, training on program Scopes of Required Services (SRS), best practices, and technical assistance to provides to ensure contracting and program requirements are met
  • Additional duties as assigned
  • Work closely with Psychiatrists, case managers, TBS Workers, Behavioral Specialists and other therapists as available to facilitate implementation of the child/family treatment plan. Maintain positive communication with all agency staff, clients and visitors.

QUALIFICATIONS:

  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrate knowledge or experience with evidenced-based case management techniques including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
  • Skill in communication, including public presentations
  • Hold a minimum of a Bachelor’s Degree and 2 years or relevant experience OR have equivalent relevant experience
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
  • A valid driver’s license
  • Auto Insurance

PHYSICAL DEMANDS:

WORKING CONDITIONS: 

This is an exempt position, with a hybrid office three (3) days in the office and up to two (2) days remote after the 90-day introductory period. This can vary based on organizational need. There may be occasional evening or weekend events.
