Job Summary:

Is the initial point of contact for all Clients and their families. Is responsible for intakes of all Veterans seeking assistance at the Catskill Veterans Outreach Center, must determine eligibility for programs and then conduct needs assessment, provide case management, and link Veterans with necessary services. Ensure that all Veterans seeking help are assisted in some way. Is the initial point of contact for community outreach, fundraising, and special events.

Job Duties:

• Perform eligibility assessments to determine that Veterans are eligible for SSVF Assistance determined by the CNYVOC, that the Veteran is homeless or at imminent risk.
• Provide holistic and comprehensive case management services to all clients: including intake assessment, benefit assessment, goal setting, long –term case management, tenant education, advocacy, and referrals.
• Identify opportunities for collaborative ventures with local apartment complexes, rental agencies, and owners of rental houses to begin the process of finding local affordable housing for Veterans.
• Ensures all applicable regulations and guidelines are followed before requesting approval for assistance.
• Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
• Help all clients and their families that are in need.
• Attend community-based meetings when needed.
• Work closely with the VA, DOL and any other government agencies and community agencies as needed.
• Complete SOAR applications for eligible clients

Requirements:

• Must be proficient with computers
• Must be able to communicate clearly and professionally both in the spoken and written word
• Strong ability to work as a team as independently
• Must possess organizational, planning, information gathering, information monitoring, problem analysis and problem-solving skills
• Must possess judgment and decision-making ability, initiative, confidentiality, and attention to detail and accuracy

Job Type: Full-time

Pay: From $30,000.00 per year
