Requirements:

• An Associate's and/or Bachelor's degree from an accredited college or university in relevant major, (i.e. Human Services, Social Work, Substance Abuse, etc.)
• At least one to two years case management experience or equivalent
• Valid driver's license

Responsibilities:

• Perform eligibility assessments to determine that Veterans are eligible for SSVF Assistance determined by the CNYVOC, that the Veteran is homeless or at imminent risk.
• Provide holistic and comprehensive case management services to all clients: including intake assessment, benefit assessment, goal setting, long-term case management, tenant education, advocacy and referrals
• Identify opportunities for collaborative ventures with local apartment complexes, rental agencies and owners of rental house to begin the process of finding local affordable housing for Veterans
• Ensures all applicable regulations and guidelines are followed before requesting approval for assistance
• Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral
• Help all Veterans and their families in need
• Attend community-based meetings when needed
• Work closely with the VA, DOL, and any other government agencies and community agencies as needed
• Complete SOA applications for eligible clients
• Input HMIS data
• Assist Housing Manager with tenant case load
• Assist Housing Manager with Apartment Inspections
