Position Title: SNF RN Supervisor

Position Summary:

As the SNF RN Supervisor, you will play a critical role in overseeing the nursing staff and ensuring the delivery of exceptional patient care within our Skilled Nursing Facility. Your strong leadership, clinical expertise, and compassionate approach will contribute to maintaining a safe, supportive, and enriching environment for our residents and their families.

Responsibilities:

Nursing Leadership: Provide strong leadership to the nursing team, ensuring adherence to established policies, procedures, and protocols while maintaining a positive and collaborative work environment.

Staff Oversight: Supervise and coordinate the activities of the nursing staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs), to ensure efficient and effective care delivery.

Resident Care: Assess and monitor the health status of residents, develop individualized care plans, and ensure the implementation of appropriate nursing interventions to promote residents' well-being.

Medication Management: Oversee medication administration, including accurate documentation and compliance with medication protocols, ensuring resident safety and adherence to state regulations.

Quality Assurance: Participate in quality assurance initiatives, conduct regular audits, and maintain compliance with relevant regulatory agencies and standards.

Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to develop comprehensive care plans and optimize residents' outcomes.

Family Engagement: Establish and maintain positive relationships with residents' families, providing updates on care plans, addressing concerns, and offering emotional support as needed.

Staff Development: Identify training needs and provide coaching and mentorship to nursing staff, fostering professional growth and maintaining competence.

Admissions and Discharge: Participate in the admission and discharge processes, ensuring a smooth transition of care and continuity of services.

Emergency Response: Act as a leader during emergencies, demonstrating swift decision-making, and ensuring appropriate protocols are followed.

Documentation: Maintain accurate and detailed electronic medical records (EMR) for each resident, adhering to facility policies and regulatory requirements.

Qualifications:

Valid and current Registered Nurse (RN) license in the state of practice.

Minimum of [X] years of experience as a Registered Nurse, with previous supervisory experience preferred.
