Description

POSITION:  Autism Waiver Service Coordinator FLSA CLASSIFICATION: Non-Exempt

STATUS: Full Time NORMAL WORKING HOURS/DAYS: M-F

LOCATION:  Family and Community Supports Division

Person-centered Focus: 

Autism Waiver Coordinators provide a wide variety of person centered supports and services with a strong emphasis on personal choice to students with Autism and their families. The Autism Waiver Coordinators serve as advocates and promote self-advocacy. They provide resources to families and coordinate supports and services with provider agencies based on the family’s preferences.

Position Summary:

The Autism Waiver Service Coordinator promotes and supports the rights of individuals with developmental disabilities and their families to be involved fully in all aspects of community life. The coordinator provides primary contact and casework services to participants on the Autism Spectrum and their families. 

Essential Functions:

Case Management:

1. Maintains good rapport and frequent communication with the people receiving services, their families and team members to ensure quality supports.

2. Responds to phone calls within 1 business day. 

3. Assists in maintaining benefits as needed.

4. Maintains a detailed, organized record for each person in the program, including at least monthly contact notes.

5. Completes quarterly visits and annual re-assessments.

6. Provides resources to families.

7. Completes service provider changes. 

8. Monitors services and treatment plans to ensure quality. 

Treatment Plan:

1. Participates fully in the development of the treatment plan.

2. Facilitates team meetings in an effective manner, resolves conflicts via consensus, advocates for the person receiving supports.

 Incidents:

1. Uses good judgment in responding to incidents. 

2. Reports incidents according to policies and procedures.

3. Provides effective follow-up.

Quality Improvement:

1. Maintains highest standards according to QA, PGCPS audits and family surveys

2. Takes appropriate steps in response to feedback. 

Mental, Physical, & Visual Demands: Effectively utilizes computer and calculator.

Working Conditions:  Normal office environment. Expected to travel throughout county/state for meetings

Requirements

Qualifications:  

A bachelor’s degree related to the human service field with at least one to three years of relevant experience in the Development Disability field. Excellent communication skills and the ability to work well in a team setting. Great documentation skills and ability to maintain records are highly desired.
