Job Title: Community Experience Specialist

Reports To: Program Coordinator

FLSA Status: Nonexempt

SUMMARY

Implements person centered plans to engage individual(s) supported in activities to either acquire new adaptive skills or support the individual(s) in utilizing adaptive skills to become actively involved in their community. Provides and seeks out opportunities for individual(s) supported to make choices. Treats all individual(s) supported, families, coworkers, and supervisors with dignity and respect.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Track Community Experience staff responsibilities; cleaning (bldg. & vehicle), probe goals, Rights & Responsibilities.
  • Maintain good time management to meet deadlines.
  • Maintain professionalism at all times.
  • Assist with establishing good rapport with community and ensure staff do as well.
  • Responsible for supervision of individuals inside and outside the center or in the community
  • Supervises assigned staff in consultation with Program Coordinator & QIDP
  • Assist with training Community Experience staff how to complete daily documentation in Therap,
  • Provide training to Community Experience on how to complete individual training goals, behavior modification and additional training that may be needed.
  • Prepares performance evaluations for assigned staff as scheduled.
  • In the absence of coordinating staff, supervises assigned staff and arranges coverage for leave request. Absences
  • Assist with developing and implementing training goals, services objectives and CE opportunities/activities.
  • Assist with developing weekly calendars and schedules.
  • Develops and facilities community connections and relations with input from individual(s) supported.
  • Train new CE staff with policies & procedures
  • Provide on-going training for current staff on
  • Models’ appropriate behavior as well as monitors and redirects individuals’ behavior including, de-escalating anger, frustration, assisting in conflict resolutions, and keeping the individual focused on activities.
  • Maintain regular communication with Supervisor regarding program enhancement opportunities and any concerns regarding operations.
  • Provides coverage to CE staff when needed.
  • Ensures the department is following HCBS compliance.
  • Assist with assessments, running reports, and maintaining accurate documents and records.
  • Be punctual and in attendance at worked as scheduled.

SUPERVISORY RESPONSIBILITIES Carries out supervisory responsibilities in accordance with the organization ‘s policies and applicable laws in consultation with Program Coordinator

EDUCATION and/or EXPERIENCE Associate’s degree (A.A.) or equivalent from two-year college or technical school; or two-years related experience and/or training working with individuals with intellectual disabilities; or equivalent combination of education and experience.
