Alpha Rehabilitation Servicesis looking for a Physical Therapist to join the team at our Skilled Nursing Facility in Brooklyn, NY!

Physical Therapist Responsibilities
- Screen, evaluate, and treat residents per MD order
- Develop effective treatment plans
- Supervisor Physical Therapy Assistants and Rehab Aides in resident care
- Communicate with Supervisor and interdepartmental personnel regarding resident progress, problems, and plans
- Record clinical documentation according to accepted regulatory, facility, and professional guidelines
- Participate in scheduling of residents and discharge planning
- Secure necessary medical equipment and adaptive equipment for residents to facilitate independence
- Comply with state regulations and national standards and Allure Rehabilitation Services policies and procedures as they apply to the practice of Occupational Therapy
Physical Therapist Requirements
- Degree from American Occupational Therapy Association Accredited School of Occupational Therapy
- Current, valid NYS license
- COVID-19 vaccination
Preferred
- Previous experience in post-acute/long-term care
