Responsibilities:
Evaluate, assess progress and provide plan of care and exercise prescription for patient caseload
Consistently ensure a world-class level of clinical care and patient experience
Document all treatments in our EMR
Communicate via chat and phone with patients and/or providers as needed
Able to commit to a minimum of 4 visits/week in either an in-home or telehealth setting
Requirements:
Licensed Physical Therapist with 1 years of clinical experience
Completed state regulated CEU requirement in New York
BLS certification
Demonstrated excellent written & verbal communication skills
Excited to build and deliver a new model for achieving better clinical outcomes
Creative and flexible, but always puts the patient first
Excellent at forging successful and respectful relationships with the entire team
Bonus: Home care or Telehealth experience
Job Type: 1099 independent contractor
Pay: Earn $60 to $75 per visit. We also offer incentives and an uncapped referral bonus program. Plus $750 sign on bonus!
