Job Summary:
We are seeking a dedicated Care Manager to join our team. The Care Manager will be responsible for coordinating patient care and providing support to healthcare professionals.

Duties:
- Collaborate with healthcare providers to ensure quality patient care
- Conduct assessments and develop individualized care plans
- Monitor patient progress and update care plans as needed
- Communicate with patients and families to provide education and support
- Coordinate referrals to specialists and other healthcare services
- Utilize medical terminology in documentation and communication
- Engage in utilization management to optimize patient outcomes

Skills:
- Proficiency in medical terminology
- Knowledge of utilization management principles
- Ability to work collaboratively with a multidisciplinary team
- Strong communication and interpersonal skills
- Detail-oriented with excellent organizational abilities

Some medical background preferred.

HOURS MAY INCREASE AS WORKLOAD INCREASES

Job Types: Full-time, Part-time

Pay: From $10.00 per hour

Expected hours: 15 – 20 per week

Schedule:

• 8 hour shift
• Day shift
• Monday to Friday
• No nights
• No weekends

Work setting:

• Clinic

Experience:

• Nursing: 1 year (Preferred)
• Case management: 1 year (Preferred)

License/Certification:

• RN (Preferred)

Ability to Commute:

Ability to Relocate:

Willingness to travel:
