Position Summary:
The Director of Clinical Operations is the bricoleur clinical leader of Trusted Care at the organization, creating and maintaining a positive culture of high performance and exemplary patient care. The Clinical Leader ensures quality, service excellence, and regulatory compliance. The Clinical Leader exemplifies the mission, vision, and values of the organization. The Clinical Leader achieves this by:
Developing and launching the clinical pathways necessary to rollout and grow a serious illness organization serving our areas in collaboration with the COO, that redefines serious illness care
Leading the day-to-day clinical care of patients and families
Providing excellent patient care experience driven by the patient care plan
Ensuring clinical operational excellence
Being a good steward of resources by managing the Big 5 clinical expenses (labor, supplies, pharmacy, DME and mileage)
Ensuring spectacular customer service for patients, families, and referral sources
Coaching, mentoring, and leading the clinical team
Ensuring service, quality and compliance pillar success through strategic operational planning, implementation, and execution and ensuring goals are achieved.
Ensuring excellent CAHPS scores through utilizing the 5 Gems
Community and referral partner outreach and relationship building in partnership with growth team.
Ensuring all clinical policies and procedures are implemented and followed
Creating a program with commitment to excellent patient care and open access to services with urgency
Ensuring regulatory compliance (state, federal, local, Medicare, SCDHEC, DEA, and accreditation standards)
Guiding and collaborating with IDG
Implementing all process improvement effort identified by the QAPI team
Building a high-performance team by intentional recruiting and hiring
Ensures all new clinicians receive stellar onboarding which promotes excellent care
Conduct monthly one to one, clinical team meetings, and supervisory visits
Seeking guidance, coaching, learning and collaboration as part of the Teleios Collaborative Network
Knowledge, Skills & Abilities:
Knowledge of federal, state, and local regulations r/t Hospice
Knowledge of Medicare Conditions of Participation for Hospice
Knowledge of quality standards and reporting practices for hospice and palliative care
Knowledge of Adult learning & training methodologies

Qualifications:
Education
Registered Nurse
Bachelors or Masters of Nursing degree– preferred
TCN Leadership Immersion
Teleios University graduate preferred
Experience:
6 years in a Leadership position in Hospice or Palliative Care
Extensive experience with leading clinical teams in hospice and palliative care
Experience and Knowledge of team formation and building
Experience of great life struggle or challenge strongly preferred
Licensures & Certifications:
Active Registered Nurse license in the state of SC
Active Professional Affiliation preferred
Valid driver’s license and proof of automobile insurance (or other mode of personal transportation) required
