A healthcare organization in New York City is currently seeking a Cantonese, Mandarin, and/or Spanish-speaking Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) to join their team in the Bronx as a Social Work Care Manager. In this role, the Social Work Care Manager will be responsible for providing psychosocial support counseling and other social work related services to Palliative Care patients and their families in both community homecare and facility settings.

Responsibilities:

The Social Work Care Manager will:

• Serve as a member of the Palliative Care interdisciplinary team (IDT)
• Provide comprehensive psychosocial report to the team
• Develop the palliative social work plans of care and the goals associated with those plans
• Make recommendations based on patient/family needs for an active caseload
• Refer patients to Hospice whenever appropriate
• Perform other duties, as needed

Qualifications:

• Bachelor's Degree
• NYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)
• Bilingual (English and Cantonese, Mandarin, or Spanish)
