Duties and Responsibilities: 

• Formulates programs for treatment and rehabilitation of clients using knowledge of current mental health diagnosis and counseling and treatment techniques.
• Counsels clients individually and/or in group sessions (as appropriate) to assist the client in overcoming individual or family issues, mental illness, or substance abuse.
• Counsels family members (as appropriate) to assist with treatment issues and to serve as a support system to the client.
• Refers clients to other supportive services as needed.
• Prepares and maintains reports and case histories.
• Assists in providing quality assurance and process improvement reports for improving quality of patient care and provides support to the Case Management Team, as needed.

Qualifications:

• Master's Degree in Counseling, Social Work, or related fieldrequired.
• LISW, LMSW, LPC or LMFTlicense in the State of South Carolina required.
• One to three years' experience in a primaryhealthcare setting preferred.
• Must maintain a South Carolina Drivers License. 
• CPR certification required.
