Under the direction of the Director of Nursing, the MDS Lead (Registered Nurse RN) is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

We are a 179-bed Skilled Nursing facility in Lutz, Florida and is designed for culture change based on the national movement for more individualized care. We focus on the individual needs of each of our patients and partner with physicians, patients, and their families to ensure successful patient outcomes. We offer a unique staffing structure with incentives. MDS LEAD REGISTERED NURSE RN QUALIFICATIONS:

• Must possess current and valid state Registered Nurse (RN) License
• Must be able to relate positively and favorably to residents, families and outside agencies and to work cooperatively with other associates
• MDS / PDPM Experience Required
