Tampa Lakes Health and Rehabilitation Center is now hiring MDS Coordinator (Registered Nurse RN or Licensed Practical Nurse LPN) who is committed to providing the best life and care for our residents. Under the direction of the Director of Nursing and MDS Lead, the MDS Coordinator (Registered Nurse RN or Licensed Practical Nurse LPN) is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.
We are a 179-bed Skilled Nursing facility in Lutz, Florida and is designed for culture change based on the national movement for more individualized care. We focus on the individual needs of each of our patients and partner with physicians, patients, and their families to ensure successful patient outcomes. If you are a Registered Nurse (RN) or Licensed Practical Nurse ( LPN ) with MDS Coordinator experience that is looking forward to becoming a part of a dynamic team, we would love to meet you! MDS COORDINATOR REGISTERED NURSE / LICENSED PRACTICAL NURSE QUALIFICATIONS:
  • Must possess current and valid Registered Nurse (RN) License or be a Licensed Practical Nurse (LPN) in the state of Florida
  • Must be able to relate positively and favorably to residents, families and outside agencies and to work cooperatively with other associates
  • Prior MDS / PDPM Experience Required
We conduct interviews in person Monday- Friday 10:30am - 4:00pm at our center
