Registered Nurse Case Manager
Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a crucial role in coordinating and managing the care of patients, ensuring they receive the highest quality of care and support. This position requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities:
- Perform diagnostic evaluations and patient assessments to develop individualized care plans
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care
- Utilize managed care principles to ensure appropriate utilization of resources
- Conduct utilization management activities to optimize patient outcomes and minimize healthcare costs
- Provide case management services, including coordinating medical appointments, arranging transportation, and facilitating access to community resources
- Maintain accurate and up-to-date medical records in compliance with HIPAA regulations
- Advocate for patients' rights and ensure their needs are met throughout the care continuum
- Collaborate with interdisciplinary teams to develop discharge plans for patients transitioning from the ICU or other acute care settings
- Provide direct patient care as needed, including administering medications, monitoring vital signs, and providing emotional support
Requirements:
- Valid Registered Nurse (RN) license in [state]
