POSITION SUMMARY
In accordance with Care Management, Discharge Planning, and Social Service policies, the employee monitors and coordinates the plan of care; identifies and evaluates patients’ individual needs; and assists patients, families and health care professionals in addressing these needs and in developing a plan for post-hospital care. The employee also serves as an advocate for patients and families and as a resource and educator for medical staff and hospital personnel.
