POSITION SUMMARY

In accordance with Care Management, Discharge Planning, and Social Service policies, the employee monitors and coordinates the plan of care; identifies and evaluates patients’ individual needs; and assists patients, families and health care professionals in addressing these needs and in developing a plan for post-hospital care. The employee also serves as an advocate for patients and families and as a resource and educator for medical staff and hospital personnel.

QUALIFICATION

EDUCATION:

• Graduate of NLN accredited school of nursing. BSN preferred.

EXPERIENCE/SKILL SETS:

• Requires recent experience in Care Management, and in similar, or in other progressive jobs. Five years experience in Med-Surg nursing, public health nursing, or discharge planning experience required. Psychiatric nursing is of additional value.
• Strong written and verbal communication skills.
• Requires proficiency in team work environments

LICENSURE/CERTIFICATION(S):

• Massachusetts state RN license required.
• Remains current with continuing education/CEU requirements
