Responsibilities:

• Deliver comprehensive nursing care to patients in a home health setting
• Conduct patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Monitor and document patient progress
• Collaborate with interdisciplinary teams to ensure coordinated care
• Educate patients and families on self-care techniques and health management
• Maintain accurate and up-to-date patient records

Qualifications:

• Valid and current Registered Nurse (RN) license
• Minimum of 6 monthsof home health experience
• Proficiency in using Oasis documentation
• Experience with HCHB electronic medical record system preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Valid driver's license and reliable transportation
