This skilled nursing facility offers short-term and long-term residents with nearby access to hospital and physician services. Its proximity and affiliation with the hospital allows for patients to receive a unique continuum of care.

We provide long-term care and respite services in addition to specialty services such as physical, occupational and speech rehabilitation.

Position Summary:

·Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. /Documents patient care given.

·Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.

·Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Minimum Qualifications:

·Licenses: Current Virginia State RN License

·BSN preferred

·BLS Certification required

·One year of Long Term Care experience preferred
