This skilled nursing facility offers short-term and long-term residents with nearby access to hospital and physician services. Its proximity and affiliation with the hospital allows for patients to receive a unique continuum of care.

We provide long-term care and respite services in addition to specialty services such as physical, occupational and speech rehabilitation.

Position Summary:

*Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. /Documents patient care given.

*Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.

*Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Minimum Qualifications:

*Licenses: Current Virginia State RN License

*BSN preferred

*BLS Certification required

*One year of Long Term Care experience preferred
