Summit Pointe is Hiring a Home-Based Caseworker (Care Manager) Near Battle Creek, MI

Pay Range: $19.25 - $28.09

Reports To: Director of Youth Services

Job Summary: Summit Pointe is the community mental health service program (“CMHSP”) for the county of Calhoun (Michigan). As a CMHSP, Summit Pointe provides supports and services to its customers addressing a wide-variety of behavioral, developmental, and mental health challenges, including, for example, psychological and clinical counseling, substance-use-disorder treatment and counseling, crisis intervention, case management, autism services, and many others. Home-Based services are a community based therapeutic service who work with youth and families with severe emotional disturbances. Services are provided within the community. Home-Based caseworkers are members of a small team comprised of a skills coach and therapist who work together to impact a youth’s goals. goals listed on the youth’s treatment plan.

Duties and Responsibilities

  • Implement a customized set of strategies, supports and services to meet the individualized goals of the youth and family.
  • Completion of psychosocial assessments
  • Coordination and monitoring
  • Education and training
  • Engages in risk management and completes safety planning.
  • Performs outreach activities within the community.
  • Maintains record compliance according to agency guidelines and standards as well as funding source regulations.
  • Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements or minimum of 24 hours per year of clinically based trainings), implementation of evidence based practices, peer review attendance, and professional improvement practices.

Qualifications and Capabilities:

  • Limited or fully licensed Bachelor’s in Social Work preferred.
  • Bachelor’s degree in a human services field is required.
  • Child Mental Health Professional, as defined by the Michigan Department of Health and Human Services provider qualification, is preferred.
  • One year of computer experience – Word, Excel, and other electronic medical record software preferred.

Essential Function:

This position will be knowledgeable about and actively support: 1) culturally competent, recovery-based practices; 2)person-centered planning as a shared decision making process with the individual, who defines his/her own life goals and is assisted in developing a unique path toward those goals; and 3)a trauma informed culture to aid individuals in their recovery process.

Trauma Informed System of Care:

A trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers. The impact of trauma is broad and touches many life domains. Participation is mandatory for activities required for Summit Pointe to become a “Trauma Informed System of Care.”
