Summit Healthcare Services is Hiring a Hospice LPN Near Phoenix, AZ

Summit has a one-of-a-kind progressive continuum of care, offering services for clients in any stage of aging, from Living Well, Home Care and Therapy to Skilled Home Health, Palliative and Hospice.

We are currently looking for a motivated Full-Time LPN to support our Hospice RN Case Managers manage their patient caseloads around the Metropolitan Phoenix Area and West Valley (visits in the home setting, care homes, and a large portion of facility-based patients).

Utilizing their depth of experience and knowledge, the home health nurse is responsible for:

  • Completing routine hospice comprehensive assessment of patient and family to determine home care needs
  • Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by a physician
  • Evaluating patient care needs
  • Administering medications and treatments as prescribed by the physician

Requirements:

  • Valid AZ LPN license (or nurse compact license) (required)
  • Hospice experience of 1-3 years (preferred)
  • Experience with Wellsky Consolo (preferred)
  • Valid AZ Driver’s License and automobile insurance (required)
  • Reliable transportation (required)
  • Pass background check (required)
