We are currently looking for a motivated Full-Time LPN to support our Hospice RN Case Managers manage their patient caseloads around the Metropolitan Phoenix Area and West Valley (visits in the home setting, care homes, and a large portion of facility-based patients).

Utilizing their depth of experience and knowledge, the home health nurse is responsible for:
- Completing routine hospice comprehensive assessment of patient and family to determine home care needs
- Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by a physician
- Evaluating patient care needs
- Administering medications and treatments as prescribed by the physician
Requirements:
- Valid AZ LPN license (or nurse compact license) (required)
- Hospice experience of 1-3 years (preferred)
- Experience with Wellsky Consolo (preferred)
- Valid AZ Driver’s License and automobile insurance (required)
- Reliable transportation (required)
- Pass background check (required)
