Health Care Coordinator / Director of Nursing (LPN/RN) for 86 unit Assisted Living Facility

Job Description::The Healthcare Coordinator (HCC): is responsible for the care of residents and coordinates their care with the rest of our team. She/he must be familiar with resident’s condition, medications, service evaluation and health assessment, family, and medical/nursing needs. The HCC supervises nurses and resident assistants. The HCC communicates with staff, resident, family, physicians, and all other disciplines to develop and follow service evaluation and health assessment. HCC must complete service evaluation and health assessment and all assigned documentation. Responds to emergency situations, including falls, injuries, fire, or disasters, and will be expected to be actively involved in working through the situation. The HCC supervises the activities of other care givers for appropriateness of care and teaches specific care for each resident as needed. Must be aware of and adhere to State and Federal regulations and Nursing Standards of Practice Act.
Requirements:
- Must have experience in dementia care.
- Must have at least 3 years management experience.
- Must be able to inspire a team approach for staff.
- Must be able to handle communications with family members.

- Salary up to $70,000, commensurate with experience.
