Memorial Hospital at Gulfport is Hiring a Transitional Care Coordinator - Driftwood LPN - Days - FT Near Gulfport, MS

The Transitional Care Coordinator is responsible for providing nursing care to the residents as prescribed by the physician and in accordance with established facility protocol, local, state and federal regulations. The Coordinator will ensure medical records follow regulatory guides utilizing the Medicare Documentation Guidelines.

Required Qualifications:
Education: Graduate from an accredited, state approved LPN program or school of nursing.

Licensure: Current Mississippi LPN or RN license, MS 90-day temporary permit, or Compact state license.

Experience: One (1) year of clinical nursing experience.

Knowledge, Skills, Abilities: Excellent verbal and written communication skills with the ability to interact or educate residents. Ability to work independently as well as with a team. Knowledge of nursing practices and procedures as well as the laws, regulations, and guidelines governing nursing functions in the long-term care facility. Knowledge of uses and operations of equipment and instruments used in patient care. Must be patient/customer service oriented, projecting professionalism and socio-economic and cultural sensitivity. Working knowledge of electronic medical records.

Preferred Qualifications:
Education: Graduate from an accredited school of nursing.

Licensure: None.

Experience: None.

Knowledge, Skills, Abilities: Proficiency with Microsoft office applications.
