PositionOverview:
The Nurse Navigator Patient Care Coordinator works collaboratively with the cancer care team to coordinate the care for cancer patients referred to the Singing River Cancer Center for all oncology patients. He/She communicates with Singing River Health System departments to expedite the care of the oncology patient from referral to treatment. He/She manages a designated clinical area with key responsibilities. The Nurse Navigator Patient Care Coordinator organizes and facilitates care for patients by adjusting staffing at a designated location based on patient needs, problem solving with staff and leading performance improvement projects.

The Nurse Navigator Patient Care Coordinator follows an individual cancer patient’s progress along the care continuum from referral or diagnosis through survivorship and intervenes as needed. He/She coordinates services to include education about the disease process, treatment options, potential side-effects, survivorship and assistance with decisions regarding end of life care. He/She assists with referrals to other specialized facilities and ensures follow-through. The Nurse Navigator serves as the primary contact for referring physicians, patients and caregivers and the liaison for access to services and resources provided through the Singing River Cancer Center.

Education:
Graduate from NLN School of Nursing required. Bachelor of Science in Nursing preferred.

License:
Currently licensed to practice as a Registered Nurse in the Stateof Mississippi.
Certifications:
Must complete BCLS certification by the end of position orientation; must maintain current BCLS certification. Oncology Certified Nurse (OCN) certification preferred; Oncology Nursing Society (ONS) Chemotherapy Immunotherapy Provider Certification Preferred.
Experience:
Two (2) years’ experience as a registered nurse preferred. Oncology experience preferred. Management experience preferred. Demonstrated leadership skills are required.
Reports to:
Cancer Center Service Line Patient Care Coordinator
Supervises:
Staff RN, LPN, MA, Certified Nurse Assistant
Physical Demands:
Work is mainly active: involves regular requirements to move about the unit, move about the facility, and to travel to another facility within the SRHS service area. Work involves exerting oneself to lift, carry, push, pull, or otherwise move objects, including the human body. Work involves using repetitive motions: substantial movements of the wrists, hands, and/or fingers while operating standard office equipment such as computer keyboard, copier, and 10‐key.
Work involves being able to perceive the nature of sound at normal speaking levels with or without correction; the ability to make fine discriminations in sound. Work requires close visual acuity and the ability to adjust the eye to bring an object into sharp focus, i.e. shift gaze from viewing a computer monitor to forms/printed material that are closer to compare data at close vision.
Must be able to be active for extended periods of time without experiencing undue fatigue. Must be able to work flexible hours, when required/requested.
Mental Demands:
Must possess keen mental faculties in the management of information. Must possess emotional stability conducive to dealing with high stress levels associated with care of acute patient/family, rapidly changing patient conditions, emotional demands of patients and families, and demands of maintaining effective working relationships with peers, managers, physicians, and other health care workers. Work requires superb communication / speaking / enunciation skills to receive and give information in person and by telephone.
Special Demands:
Must possess superior customer service skills and professional etiquette. Must possess proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.), MS Outlook and Word.
