Summary :
Ensures timely, system-wide access to comprehensive, quality healthcare for patients with diagnosis pertaining chronic disease. Facilitates timely communication between healthcare team members, collaborating with physicians, nurses, transitional care coordinators, social workers and other team members. Serves as a patient/family educator and advocate ensuring the patient receives optimal care and services throughout the transition from inpatient to outpatient, within the health care system. Provides access to information and resources to empower patients to self-manage care for 30 or up to 90 days post-discharge. Alleviates barriers to care, decreases hospital length of stay, improves adherence to treatment regimen, reduce readmissions and avoidable ED visits, and to improves patient access to care for the promotion of quality, cost effective outcomes.
Formal Education Required :
a. Graduate of an accredited school of professional nursing or a school accepted for candidacy status from an accrediting body.
b. BSN preferred; required within 3 years of hire.
c. Basic Cardiac Life Support (BLS) required; Advanced Cardiac Life Support (ACLS) preferred
Experience and Training Required :
a. Minimum of three (2) years Care Management or Transitional Care
b. Minimum of three (3) years RN experience in direct care of the cardiac patient population, either in ED, Cath Lab, CCU or Telemetry Unit preferred

Level of Physical Demands:
a. Sedentary: exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
b. Ability to walk long distances within the hospital campus.
c. Ability to sit and/or stand for long periods of time.
d. Manual dexterity to operate computer, phone, and standard office machines.
e. Ability to travel to patient homes and facilities.
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials.
