We are looking for Home Health Nurses to perform LPN and RN shifts weekly based on the Patient's preferred schedule in the comfort of their home. Particular Specifications that would be very helpful would be any experience with the following: Ventilator Care, G-tubes (gastrostomy tube), and Trach tubes (tracheostomy tube)

• Shift: Flexible, Monday - Friday, Saturday through Sunday, depends on Patient needs and your availability (including daily hours)

Qualifications

• Education: High school diploma or GED equivalent required
• Experience: RN License Required
• Car Insurance Required
• Driver's License Required
• Reliable personal vehicle Required

Responsibilities include:

• Provides home health skilled nursing services to patients through regularly scheduled home visits.
• Follows established Plan of Care (POC) and understands the process for changing the POC according to the patient and family needs.
• Documents all interventions, patient/family response, and medication administered in the medical record
• Completes patient notes, charting the same day the shift is provided
• Assists in the planning and evaluating of the nursing care for patients and their families
• Participates in performance improvement activities as requested
• Act quickly and responsibly in cases of emergency - call the office ASAP
• Undertake continuous training, cetifications as required by the state of PA

Pay: $30.00 - $35.00 per hour
