Job Title: Primary Care APRN
Department: Health Services Reports To: Vice President, Clinical Services FLSA Status: Non-Exempt (part-time) SUMMARY The Primary Care APRN provides health care to the patients of the Sulzbacher Clinics (including the Mobile Health Outreach Clinic) under the supervision of the Clinic physician. The APRN acts in accordance with the rules and regulations of the Nurse Practice Act and is fully licensed in the State of Florida. He/she actively collaborates with other healthcare providers to ensure holistic patient care, and maintains documentation and patient records in accordance with regulatory and Center policies. SUPERVISORY RESPONSIBILITIES This position has no supervisory responsibilities. ESSENTIAL DUTIES AND RESPONSIBILITIES
Maintains productivity levels while providing quality medical care to eligible patients including diagnosis, preventative care and treatment, medication management and referrals for other services as needed
Promotes positive health behaviors and self-care skills through education and counseling
Evaluates effectiveness of treatment programs on patient progress in resolving identified problems and moving towards defined objectives
Completes all relevant charting, documentation and follow-up. Maintains patient-related paperwork, including federal and state mandated forms, patient diagnostic records and progress notes
Maintains knowledge of new developments in primary health by reading professional literature, attending courses and seminars and establishing/maintaining contact with other healthcare agencies
Participates in quality assurance/improvement and risk management efforts
May coordinate or direct employee workshops, courses or training about primary health issues as appropriate
Coordinate patient care with members of the Housing Team and other Sulzbacher Clinic departments with a particular focus on integrated care.
Participate in all appropriate team meetings and required training sessions.
Maintain active, unrestricted APRN license.
Be comfortable working with individuals living on the street or in the woods as well as in a clinic setting.
Participates in other clinical or administrative projects as assigned by the Vice President or designee.
All other duties as assigned.
RESIDENT, GUEST AND INTERNAL CUSTOMER FOCUS The Primary Care APRN is sensitive to cultural, spiritual and practical needs of resident, families, guests and staff members in all interactions. He/she provides positive feedback and comments about the quality of work and effort that have been offered by patients, residents, families and coworkers, and adjusts priorities and schedules to meet specific customer needs. The Clinician reports and/or corrects problems related to safety, cleanliness and comfort of resident, patient and work areas, and follows Universal Precautions and Infection Control techniques. He/she demonstrates an understanding of their role in facility safety plans and procedures related to fire, weather and other disasters. EDUCATION and/or EXPERIENCE Qualifications
Current valid Florida APRN License
ANCC Certification
Master’s Degree in nursing from an accredited university
At least one year experience in a clinical setting
Excellent interpersonal and time management skills
Excellent written/oral communications skills
Customer service focused
Ability to pass a Level II Background Screening
Physical/Mental Demands & Working Conditions:
Transportation and valid driver’s license
Ability to work in a fast paced environment
Ability to work various shifts if required to ensure that primary care services are adequately administered
Able to travel locally
Be able to provide high quality, person centered care in a variety of settings including the outdoors
