Title: Human Trafficking Case Manager (bilingual Spanish)

Organizational Profile: S.T.R.O.N.G. Youth, Inc. is a youth, family, and community development organization that specializes in youth, gang, and gun violence prevention and intervention. Our mission is to increase safety and a sense of community belonging to youth and families across Long Island and the surrounding region. To accomplish this, we provide alternatives to high-risk behaviors for our youth and empower families, residents, and stakeholders to implement and endorse socially responsible initiatives in their communities.

Program Description: Our intervention program focuses on providing juveniles at the intersection between human trafficking and street life with the education, skills, and support they need to engage in sustainable life change. Our program includes mental health services for youth and their family, specialized services to address chronic trauma, case management services, mentoring, and experiential experiences.

Position Description: A community-minded case manager committed to making a difference in the lives of young people and empowering them to make positive life choices. The ideal candidate will have a passion for working with a population that consists primarily of at-risk-juveniles and youth that are gang associated/involved and victims of human trafficking between the ages 12-21, as well as their families. The Specialist will be primarily responsible for providing a supportive and referral services to these youth and their families.

All staff are expected to represent S.T.R.O.N.G. Youth, Inc. in a professional manner, act in a manner consistent with the mission and values of S.T.R.O.N.G. Youth, Inc., and help realize the organization’s mission.

  • Manage a caseload of youth with evidence-based and trauma-informed interventions
  • Provide victim-centered, trauma-informed services to victims of human trafficking onsite and in the field
  • Complete a comprehensive service plan conducting reviews and monitoring progress as needed throughout the duration of treatment.
  • Facilitate weekly group sessions with youth and parents/caregivers on a variety of life skills topics.
  • Obtain collateral information from participants, family members, outside treatment providers, schools, and other relevant teams in the assessment of behavioral health needs.
  • Provide intensive case management, including assessment of client needs, development of individualized service plans, assessment of eligibility for other public or community-based programs.
  • Make successful referrals to additional resources and referrals such as mentoring/peer support, integrated mental health and substance abuse, psychiatric referrals, and medical services.
  • Establish and maintain consultative relationships with schools and other agencies directly involved with the assigned youths & families.
  • Help survivors exercise their rights as crime victims within the criminal justice system and advocate on their behalf when appropriate
  • Provide support and guidance to intervention team members on cases staffed at case conferences as well as facilitate quarterly training on victim-centered, trauma-informed best practices addressing the needs of victims of human trafficking.
  • Ensure fidelity of all programming being provided to victims of human trafficking
  • Attend pertinent trainings and weekly team meetings to review and advise on the coordinated services offered to victims of human trafficking on the team’s caseload.
  • Represent the organization at SCATI and other pertinent task forces and partnership collaborative efforts in a way that advances our region’s capacity to provide trauma-informed, survivor-centered, and culturally rooted services.
  • Work with STRONG intervention teams and partnering agencies in organizing parent and community events focusing on trauma, and treatment modalities and interventions for victims of human trafficking.
  • Maintain detailed and timely records for every interaction with and about the youth.
  • Perform other duties as assigned.

Qualifications:

The Specialist should also be knowledgeable of clinical services, including chronic trauma theory and treatment, and advocacy in youth empowerment.

  • BILINGUAL IN SPANISH required
  • Graduate degree in health and human services field (ie social work, psychology) is highly preferred.
  • LMSW or license in a related human service field highly preferred.
  • Community professional with at least 4 years post-graduate experience providing services to survivors of violence, and conducting groups with individuals & families
  • Ability and willingness to provide training and professional development to staff and interns as needed
  • Knowledge of relevant community resources and must be current on news impacting the youth and families we serve, especially victims of human trafficking
  • Excellent interpersonal skills, including the ability to work independently and collaboratively in a team setting
  • Must be able to relate to youth and maintain professional boundaries.
  • Demonstrate comfort in interacting with families, community residents, and the various systems our youth and families are in contact with.
  • Ability to stay organized, assess priorities, take initiative, handle multiple assignments, meet deadlines to develop solutions to problems with limited direct supervision is a must
  • Attention to detail, ability to work under pressure
  • Commitment to excellence and a willingness to learn
  • Ability to maintain the confidentiality of records and information
  • Demonstrated computer proficiency
  • A valid NYS Driver's License and access to a motor vehicle is required

Position is guaranteed through Dec. 31, 2024.

Program Location: Western Suffolk County, New York

Days & Hours: Full-time. Must have a flexible schedule. Mainly late morning into evening hours. Weekends as needed. Will be discussed during the interview.

Salary: Commensurate with experience ($50k to $55k). Health insurance or Medical Opt-Out cash benefits, PTO and sick time pay, holidays/birthday, and 401(K) program benefits are available.
