SUMMARY

Family Service League provides services to individuals, children, and families to mobilize their strengths and improve the quality of their lives at home, in the workplace, and in the community. With more than 60 social service programs in over 20 locations, Family Service League is committed to servicing Long Island’s most vulnerable citizens. We are seeking a Part-Time Case manager who is Bilingual in Spanish or Creole to assist patrons at the Brentwood Public Library with applying for benefits, accessing services and providing linkages to additional services.

RESPONSIBILITIES

• Conduct intakes and assessments of individuals and families that present a variety of problems
• .Provide individual and family counseling
• Assist in linking eligible clients with public benefits and assist clients with applying.
• Provide linkage to more intensive mental health services.
•  Provide referrals to higher levels of care.
• Provide crisis intervention, when needed.
•  Document client contact.

QUALIFICATIONS

• Bachelor's degree in Social Work or related field. 
•  Two years’ of relevant experience required.
•  Prior experience working with individuals with mental illness and their families preferred.
•  Prior knowledge of community resources required.
•  Excellent interpersonal and verbal and written communication skills required.
•  Proficient computer skills, including Microsoft Office required.
•  Bilingual in Spanish or Creole required.
