STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 35-year history of providing team-based health care to the Denver-Metro area. From a single clinic in 1989, STRIDE has grown into a large, integrated health care delivery system serving 52,000 patients, annually, from 14 brick-and mortar clinics, including six school-based health centers and a versatile street medicine team.
The Care Coordination Supervisor provides direct supervision, training, and coaching to Care Coordinators under the general supervision of the Clinical Nurse Manager or Care Coordination Program/Project Manager(s). This role includes direct patient care time (75%) and administrative time (25%). Administrative time includes assisting with timecard review and approval, scheduling/staffing, training, progress reviews, disciplinary action, and site meetings.
Location: This position will oversee our Englewood, Conifer and all school based clinics (West clinics)
