Medical Social Work-Licensed Clinical Social Worker LCSW
Per visit, contracted position

The MSW provides direct consultation and coordination to patients in their home to facilitate positive patient outcomes, develop and implement a treatment plan in accordance with physician orders. Responsible for accurate, timely and proficient clinical documentation. Home Health Care experience a plus but not required.
